INTRODUCTION

  Thank you for agreeing to take part in this 360 Degree Assessment. The purpose of this is to provide the candidate with high quality feedback from co-workers and other significant stakeholders to enable them to strengthen their skills.

In the interests of anonymity, all your responses will be combined with other raters' responses in the person's feedback report unless you are the person's manager.

When responding to the questions, please be as honest as possible. Your responses should be based on the person’s most recent behaviour, not on observations from long ago, nor how you wish to see them perform in future.

We recommend you complete the questionnaire in a quiet spot free of distractions. Don’t spend too long on each question as your instinctive response is usually the most representative one.

The survey comprises 4 sections and should take around 20 minutes to complete.
